MISSION  

Waushara Connections is dedicated to linking older and disabled adults to benefits and services they may need to support their independence, health, and desired living arrangement.  We strive to provide a “One-Stop Shop” for information and referral to long-term care services and resources.

WHO WE ARE 

Waushara Connections, the resource center for information about aging and disability services in Waushara County, was established to serve as a link between services in the community and the people who need them.  Waushara Connections is available to individuals who have questions or concerns regarding aging, disabilities and long-term care services.  We are able to answer questions about such things as… 

v    Residential/housing options including assisted living, community-based residential facilities, and nursing homes 

v    Public benefits such as Social Security, Medicare, and Medical Assistance 

v    Caregiver support programs 

v    Respite care 

v    Transportation 

v    Meals on Wheels and senior dining centers 

v    Personal care services 

v    Housekeeping or chore services 

v    Education and support services 

v    Supported employment 

v    Home modifications/repairs 

v    Adaptive equipment

HOW WE HELP 

Ø     The Resource Center provides information and assistance to elderly individuals and to adults 18 and older with a mental, physical, or developmental disability.  The Resource Center is also a resource for caregivers, families, and friends. 

Ø     We make referrals to other appropriate agencies in the area upon your request.  If you are unable or unsure about contacting those resources on your own, we will assist you. 

Ø     We help you to get services you’re entitled to and may not be receiving.  

Ø     We offer long-term care counseling to you, your family, your friends and your caregivers. 

Ø     We empower you to plan for your future through practical prevention and early intervention programs. 

Ø     We offer a free, comprehensive, and confidential assessment of your personal situation. 

Ø     We help research hard to find resources for your particular area of concern. 

Ø     We reach out to you by offering on-site evaluations and education in your own home.  Yet, we also have a comfortable and accessible office space at the resource center if you prefer to come in.

FREQUENTLY ASKED QUESTIONS 

Ø     I have questions concerning a failing family member.  Call the Resource Center to learn about area resources or to schedule a free long-term support assessment administered by trained professionals.

Ø     Do you have information available regarding housing options for my mother?  The Resource Center has an up-to-date listing of all Waushara County housing options, complete with current address and telephone numbers. 

Ø     Is there any assistance for a person who needs help with a household task or home repair?  The Resource Center can offer information regarding community resources, including the Senior Home Repair program. 

Ø     I have a question about the new Medicare Prescription Drug program and SeniorCare.  How do I know which one will serve my needs?  The Resource Center has a variety of informational packets on each program, and can arrange for an appointment with the Benefit Specialist. 

Ø     Is there anyone available to assist with the Homestead Tax Credit?  Call the Resource Center for an appointment with the Benefit Specialist. 

Ø     I’ve become the sole caregiver for my husband who has been diagnosed with Alzheimer’s disease.  What information is available for caregivers?  Contact the Resource Center to find out about services available to support the caregiver in her role, and to learn more about the monthly caregiver support group. 

Ø     After my surgery I will not be able to prepare my own meals for a while.  Is there any help available?  Call the Resource Center to learn more about home delivered meals, and to start the registration process.  

Ø     I’m concerned about my neighbor.  She hasn’t been taking care of herself lately.  Call the Resource Center and a staff person will respond. 

Ø     I have an elder abuse situation to report and don’t know where to turn.  Call the Resource Center and a staff person will respond.

OUR PROFESSIONAL STAFF 

Our staff is trained and knowledgeable about agencies and programs that specialize in services for older adults and others with disabilities.  They have extensive experience working with individuals, families, and caregivers on a one-on-one basis. You will have access to knowledgeable professionals who are eager to assist you.   

Waushara Connections’ staff is committed to helping you identify and achieve your long-term care goals. 

FUNDING/SUPPORT 

Waushara Connections operates under an advisory committee comprised of consumers, citizens, elected officials and representatives from private and public agencies.  Waushara Connections is a collaborative effort between the Department of Human Services and Department of Aging Services and is funded through the Older Americans Act and other funds made available through Waushara County.  The Resource Center does not discriminate against individuals on the basis of age, race, creed, sex, or handicap.  Waushara Connections is committed to provide equal opportunity for all individuals to participate fully in their community.
